Part B. DECLARATION 聲明
     ________________________________ (Your Name/Company Name) (called “I/The Company” below) applies to request the password and control for the domain name _________________________ (called “Domain Name” below) from BEST-VIEW@ on  _____/______/________ (DD/MM/YYYY) (called “request date” below).

	I/The Company understands that after take back the password of the domain name, I/the company will take over the setting of the domain name. BEST-VIEW@ will not responsible to all the guarantee, breakdown, payment and the service termination (not limited to expired payment of the domain name). BEST-VIEW@ do not have any other responsibility for any payment for the domain in the future. 

	I/The Company understands that changing the Name Server setting to the configuration not specified by BEST-VIEW@, current services provided by BEST-VIEW@ (not limited to Web Hosting and Email Services, Domain / Website IP Redirection or any other related services) will no longer be available. BEST-VIEW@ do not have any other responsibility for any lost and duty cause by changing the name server setting and all pre-payment fee is non-refundable and non-transferable.


Part B. DECLARATION (Continue) 聲明(續)

　　本人/公司 明白當取回域名密碼後，該域名的設定將會由本人/公司全權控制。所有有關該域名之保證、故障、款項及服務中斷（包括及不限於域名過期付款），BEST-VIEW@將不會負責。BEST-VIEW@亦不會負責及承擔今後有關該域名之費用及款項。
　　
　　　本人/公司 明白當自行更改域名設定內之「名稱伺服器」(Name Server) 設定為非 BEST-VIEW@ 所指定或使用之設定後，本人/公司 現於 BEST-VIEW@ 之各項有關該域名之服務，包括及不限於網頁寄存、電郵服務、域名轉址及其他與該域名相關之服務，將會自動失效。BEST-VIEW@不會負責及承擔有關因名稱伺服器設定更改引致服務失效而帶來之損失及責任。所有預繳之費用BEST-VIEW@ 概不退還及不可轉讓。

Request for Domain Name password / control will take three working days. The password will be sent to the applicant by email. 此項申請需時三個工作天，密碼將會以電郵方式發送給申請人。


After completing the form, please email to info@best-view.net / fax to (852) 3544 2597 or send to Unit T, 4/F, Block 2, Goldfield Industrial Building, 144-150 Tai Lin Pai Road, Kwai Chung, NT, Hong Kong.

填妥後，請將表格電郵至 info@best-view.net / 傳真至 (852) 3544 2597 或郵寄至香港新界葵涌大連排道144-150號金豐工業大廈2期4樓T室。
